How It Works:

Fill in the Form at the end of this page.
Come and collect your sample kit, which will already be labelled. Ask for the yellow bottle at the reception desk.
Drop the sample in to the surgery - instructions will be on the pack
We send the sample to the lab
Once we receive the results, we will send them to you via text messaging.
